Although reference forms and background checks for Kentucky residents must be printed and returned by mail, you can complete the majority of the application electronically. The total application package includes:

  • Application and Fair Credit forms: Download. Save the blank document to your computer and reopen before entering any information. You can save your work and complete at your own pace.  Please refer to the Program Descriptions to complete the Volunteer Interests section.
  • Completed reference forms: Download and print three copies.
  • A Summary of Your Rights Under the Fair Credit Reporting Act: Download (for your records; no signature or return required).
  • Central Registry Check (Kentucky residents only): Download and mail to Volunteer Program, 4192 North Wilderness Road, Mt. Vernon, KY 40456
  • Youth Leader Request (Kentucky residents only): Download and mail to Volunteer Program, 4192 North Wilderness Road, Mt. Vernon, KY 40456

*Fair Credit Report is not a financial credit history, but rather gives CAP permission to obtain criminal and driving records from a national reporting agency.

To electronically sign your application and Fair Credit forms, you will have to create a password-protected electronic signature through Adobe. If you have never done this before, Adobe will take you through the process step-by-step. Just click on the signature field to get started. Be sure to save a copy of your completed application for your records.
