Although some application materials must be printed and returned by mail, you can complete the majority of the application online. The total application package includes:

Please note that you must complete the entire form in one sitting. If you navigate away from the page or press “submit” before completing the application, your information may be lost. Review the application before beginning to ensure you have the required information.
- Personal essay (long-term applicants only): Submit online
- Completed reference forms (you will need to print three copies): Download
- Fair Credit Reporting Act: Download (return only page requiring a signature)*
- Fair Credit Authorization Form (part 1): Download (for your records; no signature or return required)
- Fair Credit Authorization Form (part 2): Download
- Required signatures: Download
*Fair Credit Authorization is not a financial credit history, but rather gives CAP permission to obtain criminal and driving records from a national reporting agency.
